If you do not see your plan listed above, call your carrier using Tax ID: 84-2382599 and NPI: 171539192 and ask if we are in network. Many plans use provider networks like Aetna or Cigna under a different name, which is often noted on the back of your insurance card.

We are not currently in network with straight Medicaid without a HMO assignment, Florida Healthcare Plans (FHCP), Community Care Plan Medicaid, and ComPsych EAP.
